Ventricular Assist Device (VAD) Rehabilitation Program CarePartners Rehabilitation Hospital’s Ventricular Assist Device (VAD) Program is a comprehensive and holistic program that focuses on functional independence, education and device management for VAD patients and their caregivers. Rehabilitation team members work closely with the VAD team at Mission Hospital to ensure ongoing communication regarding the patient’s recovery and progress, and to enhance the transition of care between the acute and post-acute levels of care. Transition from acute care to the rehabilitation hospital is facilitated by a member of our Post-Acute Clinical Specialist team. Members of the Mission VAD team visit patients frequently at CarePartners Rehabilitation Hospital and are available 24/7 to assist our team with any change in the patient’s medical status during their rehab stay. The CarePartners VAD program maximizes the quality of life for our patients by addressing each person’s unique needs and abilities. Our staff have received extensive training in the care of VAD patients. Our team includes: Board-certified rehabilitation physician Rehab nursing staff Physical therapist Speech therapist Psychologist Chaplain Case manager Occupational therapist Respiratory therapist Dietitian Pharmacist Post-acute clinical specialist Other on-site services include, but are not limited to the following: lab, hemodialysis, radiology, wound care and animal-assisted therapy. OUTCOMES: 37 patients participated in CarePartners VAD program between January 2019 and July 2020. The average stay was 10.8 days. 81% percent of those patients were able to return to a home environment. 96% of all patients discharged during this timeframe responded “Good” or “Very Good” when asked about the overall care at CarePartners Rehabilitation Hospital.
