Diabetes Education and Management

If you are diagnosed with diabetes, Mission Diabetes Center is here to help. We offer education and support for both adults and children diagnosed with prediabetes or diabetes, which includes type 1, 2 and gestational diabetes. Our program at Mission Diabetes Center is continually recognized by the American Diabetes Association, and many of our staff are nationally-certified diabetes educators.

Diabetes Self-Management

We offer one-on-one consultations in addition to a 5-week diabetes self-management course covering the things you need to know in order to manage your diabetes and make the right self-care decisions. We also offer medication review consultations provided by our pharmacist.

Nutrition Education

These one-on-one consultations with our registered dietitians are designed to meet your specific nutrition needs and help you make lasting lifestyle changes whether you have diabetes or prediabetes. In addition to diabetes, our team of dietitians provide education for other nutrition related conditions. Click here for any blogs we have on the topic of diabetes. 

Gestational Diabetes Education

These one-on-one sessions are designed for women diagnosed with diabetes during pregnancy. The sessions include an overview of gestational diabetes, instructions on how to monitor your blood glucose levels and meal planning tips for balanced nutrition.

Insulin Pump Training and Continuous Glucose Monitoring

These consultations offer individual education and instruction for adults and children who are beginning insulin pump/ continuous glucose monitoring therapies.

We also offer professional continuous glucose monitoring. This is designed to provide better insight into daily glucose management for 7 to 14 days. This allows our providers to identify patterns and recommend medications and/or lifestyle changes.

Support Group

Mission offers a free support group each month for adults with diabetes. This support group provides the opportunity for people to interact with others who are dealing with similar health concerns.

If you are diagnosed with diabetes, let Mission Diabetes Center support you in self-management. Ask your doctor to refer you for an appointment. A physician referral is required for the program.
