The Diabetes Center team includes nurses, dietitians, and pharmacists. This specialized team works together to achieve one goal – helping our patients manage their diabetes so they lead healthy lives. Many of our team are also nationally-Certified Diabetes Educators. The Diabetes Center offers our patients a collaborative approach coordinating care both in and out of the hospital. The Diabetes Center does require a physician referral.
