Heart Path Cardiac Rehabilitation

Heart Path is a state and nationally certified comprehensive program of cardiac rehabilitation. It will help you look at the risk factors that cause heart disease and design a plan of care to lower your risks for future problems with your heart.

Heart Path Provides:

  • A medically supervised exercise program that will help you return to your daily activities and decrease your risk for heart disease
  • Education about your personal risk factors
  • Your loved ones are invited to take part in education, group support, and consultations
  • A personal recovery plan – Heart Path staff, will help you set goals to improve heart health
  • Communication with your physicians as needed

The Heart Path Cardiac Rehabilitation program brings about life-changing results by helping participants adopt and sustain lifestyle changes in three areas of their lives: what they eat, how active they are, and how they respond to stress. By optimizing these three elements, the program empowers them to minimize and avoid future surgeries and, most importantly, retake control of their health.

Who Can Be Successful in the Program?

Anyone. The question that we hear most often from potential participants is, “Can I do it?” The answer is yes. Our participants adhere to the program at a very high rate because once they enroll, they start feeling so much better, so quickly, and cannot imagine returning to their old ways of living.

Coverage

Medicare covers the program under “Intensive Cardiac Rehabilitation”.

Qualifying criteria for the program includes:

  • Acute myocardial infarction within the last 12 months (heart attack)
  • Coronary artery bypass surgery
  • Current stable angina pectoris (chest pain)
  • Heart valve repair or replacement
  • Percutaneous transluminal coronary angioplasty (balloon) or coronary stenting
  • Heart or heart-lung transplant
  • Heart Failure Stable, chronic heart failure (HF) with left ventricular ejection fraction (LVEF) of 35 percent or less and New York Heart Association (NYHA) class II to IV symptoms despite being on optimal heart failure therapy for at least six weeks; stable is defined as no recent (less than or equal to six weeks) or planned (less than or equal to six months) major cardiovascular hospitalizations or procedures
