Perinatal Navigator

Pregnancy can be exciting. High risk pregnancy can be stressful to the patient and their family. Whether a maternal or fetal diagnosis, it is often overwhelming and challenging to follow all the recommendations and referrals offered by their physician/provider.

Supporting before, during and after delivery via:

  • Introductory conversations to introduce services and to understand patient/family communication preferences
  • Tour and specialist appointments
  • Internal team familiarized with patient through regular multi-disciplinary rounds
  • Regular touchpoints with patient to answer questions and concerns
  • Answer any questions or concerns about delivery
  • Follow up after delivery with specialists or other needs

The Perinatal Nurse Navigator is an additional layer of support to provide ease for the journey by providing confidence, coordination of services and comfort during a complicated and emotional time.

Diagnosis for Perinatal Navigator Support

  • Diabetes
  • Hypertension
  • Prior Pregnancy Complications
  • Prior Preterm Birth
  • Maternal Cardiac Conditions
  • Advanced Maternal Age
  • Morbid Obesity
  • Placental Anomalies
  • Fetal Anomalies (regardless of viability)
  • Prior Pregnancy Loss

Any other extenuating circumstances in which patient could benefit from an additional layer of support and education.
