Ambulatory Services

Mission Hospital Outpatient Clinical Pharmacy Services is staffed by clinical pharmacists who provide excellent pharmaceutical care by enhancing patients' understanding of their medications and chronic diseases and who promote overall health and well-being through collaboration with patients, their families, and their health care providers. The Outpatient Clinical Pharmacy Services focus on medication therapy management and education, and they facilitate continuity of care by assisting with medication access, adherence, and reconciliation during transitions in care. Additionally, Mission's clinical pharmacists provide panel management, complex drug therapy management by referral and transitions of care support for several Mission-owned primary care and specialty clinics throughout our region.

The Outpatient Clinical Pharmacy Services provide support in the following areas:

  • anticoagulation
  • heart failure
  • geriatrics
  • chronic disease management & education
  • primary care & population health

Medication Assistance Program (MAP) provides medications, education, and disease state management to low-income patients. The objective of MAP is to improve clinical, economic, and quality of life outcomes for their patients. MAP has been able to demonstrate a reduction in hospital inpatient and outpatient admission for patients enrolled in the program. MAP was honored in 2012 with recognition as a Best Practice in Care Transitions by the American Society of Health System Pharmacists and American Pharmacists Association. Residents will receive exposure to many chronic diseases including, depression, diabetes, COPD, heart failure, hypertension, and hyperlipidemia. Residents are frequently asked to participate in community screenings and health fairs. Skills gained will include exposure to obtaining medications through the pharmaceutical industry, working with low-income patients, performing comprehensive medication reviews, and dispensing medications in a MAP pharmacy.
