Palliative Care

The CarePartners Palliative Care team provides specialized medical care for patients who are living with serious illnesses. Regardless of your diagnosis, level of treatment or age, we can help you find relief from the symptoms, pain and stress associated with having a serious illness.

Our doctors, nurses, social workers and chaplains work with your other doctors to provide an extra layer of support throughout your illness, which can mean many things, including:

  • Expert management of pain and symptoms
  • Facilitating communication between health care providers
  • Creating a care plan based on your goals
  • Emotional and spiritual support
  • Coordination of care across settings, in the hospital and the community
  • Palliative care is appropriate at any age and at any stage in a serious illness, and can be provided together with curative treatment.

Palliative Care is a consultation service requested by your physician or health care provider. Here are some guidelines on how to request Palliative Care:

  • In the hospital: Patients at Mission Hospitals can request help from our team. Ask your physician, case manager or discharge planner to call 828-277-4805.
  • In a nursing home or long-term care facility, or receiving care from a home health agency: Ask your doctor to call 828-277-4805.
  • Receiving outpatient care: Ask your doctor to call 828-277-4805.
